    I penning down this letter for [XYZ], who comes to me for his regular checkup. I am writing this message to ask for ten days of work relaxation for Anna from work as she wants complete rest amidst bad health.

    She has a large stone in her gall bladder that often results in severe pain. On [mention date] she is having a small surgery. To regain good health she needs a few days’ rest. For your reference, his medical history is attached to this note. She does not have any other issues apart from this and hopefully, she will recover soon.
