    Hi, it is to inform you that my patient is suffering from a fever and sore throat. He is not able to talk properly and walk around. In this condition, I advised him to take rest properly and avoid any type of stress. A high workload can affect his health and may worsen the situation. I am prescribing some medicine that helps him in getting better. It may take two or three days in recovering from a fever, and complete bed rest is mandatory. It is requested you grant him leave for three days or so. I am very obliged to you.

    Sir, it is to bring to your notice that my patient met an accident last night. The accident was very severe and causes him many injuries. The upper limb and thigh are fractured ad are not moving currently. His whole body is in pain and is not responding. It takes more than a 2-week incomplete recovery. I have given him medicines and advised him to have complete bed rest for at least one week or so. He is not in a condition to work properly I request you to kindly give him leave for one week or so. I am very thankful to you for this act.

    Hi, it is to notify you that my patient is suffering from extreme stress and depression. He is in a sub-conscious state for over a month and is not taking care of himself. The mental health of my patients is falling and becoming worse. He is not able to focus on work and is lacking behind in every aspect. By observing his behaviors and condition, I advised him to have complete bed rest and a proper diet to improve his performance eventually. Therefore, I request you grant him leave for three days or so. It will help him to recover soon.

    Hi, this is bringing to your notice that my patient is suffering from the upcoming epidemic of Spanish Influenza. I have done his test yesterday and the report is showing the symptoms of this disease. He is not able to stand and talk due to this worsening situation. This disease has affected him very much, so I advised him to take complete bed rest. Some painkillers and sedatives have been given to restore his health, but rest is mandatory for him in this condition. So, for his betterment, I request you to kindly give him a leave of one week. I will be very obliged to you.
