I am XYZ, the father of John Doe, who studies in grade 8 at your school. I’m writing to request a 2-week leave for John on account of his mental health. He has Borderline Personality Disorder (BPD) and has been experiencing extreme mood swings for the last few days. It is, therefore, best to let him take a break from school and social interaction for a while.
John undergoes two therapy sessions every month, along with medication. However, lately, he has started showing alarming behavioral patterns due to the stress of his studies. His doctor has advised him to allow him to take a break until his condition stabilizes.
I am attaching John’s medical reports and doctor’s prescription for details. I would appreciate your support and cooperation.

I want to request you to approve leave for my son, XYZ, of grade 5, due to his mental health issues. As you already know, he has autism. He needs a break from school on the advice of his doctor.
Although XYZ has been doing well, lately, he has been withdrawn and depressed. I would appreciate it if you could ask his teachers about a possible trigger that could have changed his behavior. We want to find out the cause so that his counseling can be done accordingly.
We believe a week-long break will help him recover. He will also be receiving therapy sessions during this time.

I want to request leave for my child formally, Jane Doe, who studies in grade 7. She has been suffering from Post-Traumatic Disorder after the passing away of her uncle. She shared a very strong bond with him and is grieving the loss. She, therefore, needs a few days off to deal with her mental state.
We would be seeking professional help for her. We would also request you to show consideration towards her once she returns to school. She needs the support of her teachers during this difficult time.
