A medical certificate is proof that a person has undergone medical treatment. It also enables others to know about the medical condition of a person. This certificate is generally issued by a medical practitioner who claims to have a deeper understanding of a person’s medical condition.

People often contact their physicians by writing them a request letter in which they humbly ask them to issue them a medical certificate. In this letter, they mention the reason for which they need the certificate because many doctors tailor the certificate to the needs of the patient.

Follow these tips for writing a professional-looking request letter:

Give your introduction:

A doctor has so many patients to deal with. Therefore, he often forgets the name and medical conditions of people. Therefore, you should give your introduction and a little background on your treatment

Make a humble request:

Ask the doctor to provide you with the medical certificate in a courteous way. Always keep in mind that your doctor is a busy person who will have to make time for issuing you the certificate. Therefore, stay humble.

Mention the reason:

Give the reason as to what has compelled you to ask for the issuance of the medical certificate.

Show gratitude at the end:

I am (mention your name), who visited you last week due to pain in my kidneys. You conducted my tests and diagnosed that I was going through acute kidney disease. As per your instructions written on the medical prescription, I was on complete bed rest. I followed all your instructions religiously. Due to your instructions, I remained on bed rest for 5 days and was absent from work.

Now, when I am trying to apply for medical leave, they are asking me to provide them with proof of my sickness. Although I submitted test results and prescriptions, they require a medical note written by my physician.

I humbly request you to issue me a medical certificate by mentioning my name in it so that I can get approval for my medical leave at the workplace. I would be very much thankful to you for the support and cooperation from your side. For reference, I am attaching the prescription issued by you with this letter.

Sub: Application for issuance of a medical certificate

With due respect, I would like to request you issue me a medical certificate. I am (mention your name), resident of (mention the location). I am a patient with chronic kidney disease who has been taking treatment from you for the last 5 years. Recently, I was not feeling well and visited you before the appointment time. After seeing my condition, you asked me to take a rest for 10 days.

I am a student of an engineering university and we are strictly forbidden to take such a long leave unless we have a genuine reason. Although I have a genuine reason, I don’t have any proof. Therefore, I am writing this letter to you to ask for the medical certificate. 

With the help of this medical certificate, I will be able to get approval for the medical leave I have applied for. This certificate will also be very helpful for me in a number of ways. Therefore, it is a humble request to you to please write me a certificate as soon as possible. I shall be very thankful to you for this act of kindness.

For making it easy for you, I am attaching my prescription with this letter. I am looking forward to a kind response from your side.
