This is to bring to your attention that I have recently been diagnosed with diabetes. After evaluating my health condition, my physician has strongly advised me to manage my physical activity levels and minimize prolonged exposure to harsh weather conditions. Therefore, I respectfully request a reduction in my fieldwork duties.
My current duties involve working on-site with unpredictable schedules. Given my condition, I have been advised not to overexert physically since my body is unable to regulate my energy levels efficiently. Overexertion may lead to hypoglycemia – a condition in which my blood sugar levels can drop significantly and may create a life-threatening situation.
Considering the issues mentioned above, I request that you make the necessary adjustments to my work duties. I assure you that I will continue to work with complete commitment and dedication.
Thank you for your consideration and support.

I am writing to request that you make some essential accommodations due to my health condition. Since I am diabetic, I need to consume frequent small meals to ensure that my blood sugar remains stable. I, therefore, request small meal breaks every 2 to 3 hours.
I need to consume healthy snacks, such as fruits, nuts, salads, or sugar-free biscuits, every few hours. I assure you that each of my meal breaks would not extend beyond 10 minutes. I will do my best to avoid disrupting the workflow or causing any inconvenience to my team members.
Your support and cooperation would be highly appreciated.

I want to request flexibility regarding the office policy prohibiting the consumption of food within the working area. Since I am suffering from diabetes, I need to consume small snacks every few hours. This helps me maintain my blood sugar levels.
It’s not a practical idea to head to the office cafeteria every time I need to eat a small snack. It disrupts the workflow and breaks my tempo. I, therefore, request permission to keep some healthy snacks, including nuts, diabetic cookies, and granola bars, at my desk. That way, I can quickly grab a snack without disrupting any ongoing tasks.
Please note that the requested snacking is a medical necessity. I would be obliged for your support and cooperation.
I appreciate your consideration.

I am writing to inform you that I have been diagnosed with Type 2 diabetes. I respectfully request an adjustment to my current work plan.
My condition requires careful and constant management throughout the day, including frequent meals, insulin administration, and frequent washroom breaks. Therefore, it would be more convenient for me to work from home. This way, I will be able to manage both my work and my health.
Please rest assured that I will perform my work duties with complete dedication. I am also willing to visit the office as needed for important meetings or other tasks that require my presence.
I would appreciate your cooperation and support.

This is to bring to your attention that I am diabetic and need quick access to healthy snacks to avoid blood sugar fluctuations. Could I please keep some healthy snacks at my desk?
I will make sure that I store the snacks neatly and discreetly. It is a medical necessity, and I will not let it disrupt my work.
Looking forward to your positive support.
